Start with Florida Courts, not a generic forms list
Florida Courts publishes statewide family-law forms and instructions explaining the stated use of each document. Start there, confirm the title and revision of every PDF, and then check the clerk or court for the county where the case will be handled.
Source review: October 4, 2026. DIY Florida describes a free interview-based program for certain official court forms through the E-Filing Portal. Standard filing and convenience fees may still apply.
First research the petition descriptions
The four guides below organize titles, printed criteria, fields, and cautions in official packets. They are not a questionnaire and do not recommend a petition.
Form 12.901(a): joint petition for simplified dissolution
The instructions list criteria involving agreement, children and pregnancy, assets and liabilities, alimony, signing, and final-hearing conditions. Review every criterion.
Form 12.901(b)(1): petition involving dependent or minor children
Review the sections concerning children or pregnancy, property and debt, spousal support, parenting, child support, requested relief, and companion filings.
Form 12.901(b)(2): petition involving property but no dependent or minor children
Review the packet's statements about children and pregnancy and its sections for property, liabilities, spousal support, agreements, and relief.
Form 12.901(b)(3): petition with no dependent or minor children or property
Review the fixed statements concerning children, pregnancy, marital property and debt, and spousal-support rights before making any decision.
Case-opening and identity forms
Official packets may discuss documents that open the court file, identify related matters, provide protected identifying information, or prove a fact. Which documents apply is a case-specific question.
Form 12.928 family court cover sheet
Review the case style, action, case-type, related-case, and certification fields plus the official filing cautions.
Form 12.900(h) notice of related cases
Review the official related-case criteria, case-detail sections, coordination choices, continuing duty, and service certificate.
Form 12.902(j) notice of Social Security number
Review the sensitive-information fields, confidentiality warning, oath, signing, and current-procedure cautions.
Form 12.902(i) corroborating-witness affidavit
Review the residence-related statements, witness oath, identification, notarization, and dated instruction language.
Financial disclosure, property, and settlement documents
Do not treat a document name as proof that it applies. Read the petition instructions, current family-law rules, and the instructions attached to each financial or settlement form.
Short-form versus long-form financial affidavit
Compare Forms 12.902(b) and 12.902(c), their printed income labels, information sections, and current revisions without having the site select one.
Form 12.902(b) short financial affidavit
Organize the case, income, deduction, expense, asset, liability, and contact information displayed by the official short form.
Form 12.902(c) long financial affidavit
Prepare for the more detailed employment, income, expense, asset, liability, contingent-item, and contact sections.
Form 12.932 mandatory-disclosure certificate
Review the temporary-relief and longer disclosure lists, service dates, privacy warning, declaration, and certificate fields.
Form 12.902(f)(3) simplified marital settlement agreement
Review this form's fixed agreement language and organize the asset, debt, contingent-item, additional-term, and contact information it displays.
Form 12.902(k) joint verified waiver
Review the specific disclosure statements, notice, timing language, child-support exception, declarations, and service certificate.
Children, parenting, jurisdiction, and support documents
A divorce involving children can raise separate questions about jurisdiction, parental responsibility, time-sharing, support, health insurance, and financial information. These guides describe form structure; they do not propose terms or calculate support.
Form 12.902(d) UCCJEA affidavit
Organize child residence history, other cases, custody claims, and related disclosures without determining jurisdiction.
Form 12.995(a) standard Parenting Plan
Review the child, schedule, holiday, transportation, travel, school, communication, and signing sections without recommended terms.
Form 12.902(e) child-support guidelines worksheet
Review the current chart and printed calculation sequence without having the site choose inputs or calculate a legal result.
Form 12.902(l) affidavit of income for child support
Review the monthly-income, waiver, oath, and service fields for the circumstances stated in its own instructions.
Response, service, and continuing contact information
Documents used after a petition is filed involve procedural rights and deadlines. Do not sign a waiver, admit statements, calculate a response date, or select a service method without reading the exact document and current rules.
Form 12.903(a) answer and waiver
Read the official admission and waiver language before reviewing its caption, copy request, service, oath, contact, and notarization sections.
Form 12.915 current mailing and email address
Review contact fields, email designations, certifications, and filing and service instructions.
Form 12.912(a) military-service memorandum
Review the verification-request fields and certification language without determining military status or service consequences.
Form 12.912(b) affidavit of military service
Review the printed status choices, factual-source fields, oath, and notarization section without making a status determination.
Information to organize after selecting a form
After independently identifying a form and reading its instructions, gather only what that form requests. Depending on the document, that may include:
- Full legal names, addresses, contact information, county, and judicial circuit
- Marriage, separation, and Florida-residence facts requested by the petition
- Case numbers and information about related court matters
- Income, expenses, assets, liabilities, and supporting financial records
- Children's identifying and residence-history information requested by a child-related form
- The factual selections and requested relief you have decided to state
- Local clerk instructions for fees, copies, submission, scheduling, service, and confidential information
Do not guess. Sun State transfers customer-supplied entries into selected PDFs; it does not resolve missing facts, draft allegations, choose relief, or review legal sufficiency.
Statewide forms do not replace local instructions
Florida Supreme Court Approved Family Law Forms are statewide, but a clerk or court may publish local procedures, administrative orders, scheduling directions, filing methods, or additional administrative materials. Confirm the current process directly with the appropriate clerk or court.
Optional fillers for forms you already selected
The official forms are free. Sun State offers paid software for certain customer-selected forms. The tools do not recommend a form, create legal strategy, file with a court, serve anyone, or replace signing, notarization, and local-court steps.

Common questions
Is there one Florida divorce packet for everyone?
No single list on this page applies to every case. The petition and supporting documents depend on facts, requests, applicable rules, and local procedures.
Can this guide tell me which petition to file?
No. It organizes official titles and links for research. A court self-help center may provide permitted procedural assistance, and a licensed Florida attorney can give legal advice.
Are the official Florida divorce forms free?
Florida Courts makes its blank forms available without charge. DIY Florida also describes free interview-based document preparation for supported matters, although filing and convenience fees may apply.
Does completing a PDF file the case?
No. Completing a document is separate from signing, notarization, filing, payment, service, disclosure, scheduling, and hearing requirements.
