What information do both forms organize?

Both official PDFs organize financial information into categories. The exact questions and level of detail differ, so review the actual form you selected. Common categories include:

  • Case caption and party information
  • Employment and income entries
  • Deductions and monthly expenses
  • Assets and liabilities
  • Certification and service information

This list describes fields found in the forms. It does not determine what you must disclose, how you should characterize an item, or what amount belongs in a field.

Why the long form takes more time

The June 2025 long form contains more detailed income, expense, asset, liability, and contingent-interest sections. Sun State estimates approximately 45–75 minutes for data entry into its configured long-form filler, compared with approximately 25–40 minutes for its short-form filler. Actual time depends on the information you choose to enter and what records you have available.

Prepare before opening a filler

Reading the official instructions first can reduce interruptions. Based only on the fields displayed in these PDFs, it may help to have your case information and the records you plan to use nearby. Leave a software question unanswered when you want the corresponding configured field to remain blank.

  • The current official PDF and its attached instructions
  • Your case number, circuit, county, division, and party names, if available
  • The income, deduction, expense, asset, and liability figures you intend to enter
  • Any supporting schedules or records you plan to consult
  • Current local filing and service information from the appropriate clerk or court
