Florida Form 12.902(e): Child Support Guidelines Worksheet

Form 12.902(e) organizes the printed child-support guideline calculation. This guide maps the current court packet, chart, inputs, arithmetic directions, time-sharing section, filing notice, and deviation choice without calculating support or deciding which entries apply.

This guide does not calculate or recommend child support.Sun State does not select income figures, classify costs or credits, determine time-sharing percentages, apply statutes, choose a deviation, predict an award, or provide legal, financial, or tax advice.

Florida Courts currently lists Florida Family Law Rules of Procedure Form 12.902(e), Child Support Guidelines Worksheet, revised June 2025. The official PDF contains 15 pages: three pages of instructions, five pages of the guideline chart, and a seven-page filing packet.

Source checked October 2, 2026. The current Florida Courts download matched the repository's reviewed official source exactly. Because the instructions warn that chart amounts can change, always confirm the revision before using a saved copy.

When do the official instructions describe using the worksheet?

The June 2025 instructions say to complete the worksheet when child support is being requested. They distinguish between knowing the other party's income—when the worksheet should accompany the financial affidavit—and not knowing it, when the worksheet is completed after the other party files and serves a financial affidavit.

This restates the packet; it is not a form-selection decision. This guide cannot determine whether child support is at issue, whether income information is known or complete, when the worksheet is due, or which filings a particular case requires.

What is inside the 15-page packet?

PagesPrinted material
Instructions 1-3Use, filing, service, e-filing, e-service election, confidential-address notice, monthly conversions, deviation reference, and nonlawyer disclosure
Instructions 4-8Child Support Guidelines Chart for combined monthly available income from $800 through $10,000 and one through six children
Filing packet 1Notice of Filing, certificate of service, recipient information, and party signature/contact fields
Filing packet 2-6Gross-income reference and worksheet lines 1 through 21
Filing packet 7Deviation requested/not requested choice and nonlawyer-assistance fields

1. Treat the chart as revision-specific source material

The five-page chart crosses combined monthly available income with columns for one through six children. The printed range begins at $800 and ends at $10,000. The instructions say chart amounts change from time to time and direct the reader to use the most recent version.

The packet separately states that when combined monthly net income is not listed in the chart, child support is calculated as provided by law in section 61.30(6), Florida Statutes. This guide does not extrapolate the chart, apply the statute, or supply an amount outside the printed table.

2. Convert recurring figures to monthly amounts

The packet's conversion examples say: twice-per-month payment × 2; every-two-weeks payment × 26, then ÷ 12; and weekly payment × 52, then ÷ 12. Those are the official arithmetic sequences, not a decision about which source number, period, or rounding method is correct.

3. Carry the financial-affidavit inputs without reclassifying them

The worksheet first asks for present gross monthly income from line 17, Section I of Form 12.902(b) or (c). Line 1 then asks for present net monthly income from line 27 of the applicable financial affidavit. Columns separate petitioner, respondent, and total values.

Sun State does not choose between the short and long affidavit, verify either source line, resolve inconsistent affidavits, impute income, or decide what counts as income or a deduction.

4. Lines 2-9 build the printed basic-obligation path

Line 2 uses the number of common minor children and the total from line 1 to identify a chart amount. Line 3 divides each parent's line 1 amount by the total to display percentages. Line 4 multiplies the line 2 amount by those percentages.

Lines 5 and 6 address the displayed child-care, children's health-insurance, and noncovered medical, dental, and prescription-medication costs and allocate their total by the line 3 percentages. Lines 7 and 8 show payments or credits actually made. Line 9 directs the reader to add lines 4 and 6 and subtract line 8.

Printed arithmetic is not legal or factual validation. This guide does not decide which children, costs, payments, credits, percentages, or insurance amounts belong in the calculation.

5. Lines 10-21 are the packet's substantial time-sharing section

The form labels this section “Substantial Time-Sharing (Gross Up Method)” and says to complete lines 10 through 21 if each parent exercises at least 20 percent of the annual overnights, identified on the form as 73 overnights.

The section increases the basic obligation to 150 percent, allocates it by the line 3 percentages, converts each parent's annual overnights into percentages, cross-multiplies support and the other parent's overnight percentage, repeats the additional-support and credit sections, and compares the resulting transfer amounts on lines 19 through 21.

This guide does not count overnights, determine whether the threshold is met, resolve schedules, choose a time-sharing method, or calculate the presumptive amount.

6. Keep the Notice of Filing separate from the worksheet

The filing packet begins with a Notice of Filing stating that the named person is filing the attached worksheet as Exhibit 1. It contains its own caption, service-method choices, service date, recipient information, and party signature and contact fields.

Completing the worksheet does not file or serve either document. This guide does not choose recipients or delivery methods, determine a service date, or certify that service occurred.

7. The deviation choice points to a separate motion

The last page says that when either parent requests an award above or below the guideline amount, Form 12.943, Motion to Deviate from Child Support Guidelines, must be completed and filed. Form 12.902(e) then provides two choices: deviation requested with Form 12.943 attached, or deviation not requested with that motion not attached.

Sun State does not recommend a deviation, state whether grounds exist, select a choice, draft a motion, or evaluate what a court may award.

8. Review filing, privacy, and nonlawyer instructions independently

The instructions discuss filing, service, Rules 2.516 and 2.525, Forms 12.914 and 12.915, and a confidential-address warning that points to Form 12.980(h). The last worksheet page also includes the required identifying fields for a nonlawyer who helped complete the form.
