Florida family-law form preparation

Florida Form 12.902(c) preparation checklist

Organize the information displayed in the Florida Family Law Financial Affidavit (Long Form) before you begin entering it.

This checklist does not select a form or provide legal advice. It describes fields in the official June 2025 form. Read the complete instructions and follow any direction from the court or a licensed Florida attorney.

Start with the free official form

The June 2025 Form 12.902(c) contains three instruction pages followed by a 15-page financial affidavit. Its instructions describe the long form as generally used when a family-law case requires a financial affidavit and individual gross income is $50,000 or more per year, while also listing exceptions. Do not use the income statement alone to select your form.

Source freshness: the Florida Supreme Court has announced family-law form title changes effective October 1, 2026, and stated that the forms remain valid during the transition. Check the current forms index before starting. Read the official amendment opinion ↗

Why this form takes time to prepare

The long form asks for more detail than Form 12.902(b). It covers employment and retirement information, last year’s gross income, present monthly income and deductions, several groups of monthly expenses, assets, debts, net worth, contingent items, and child-support worksheet information.

Sun State estimates approximately 45–75 minutes for data entry into its configured filler. Your time will depend on the records you have available and the entries you choose to make.

1. Case, employment, and retirement information

Have the identifying and work information you intend to enter nearby:

  • Judicial circuit, county, case number, division, and party names
  • Your full legal name, age, and occupation
  • Employment status, employer contact information, pay rate, and pay frequency
  • Information about additional jobs or an expected employment change, if you plan to use those fields
  • Retirement details, an anticipated retirement date, or deferred-compensation plan information, where applicable
  • Last year’s gross income figures requested by the form

2. Present monthly income and deductions

The official form requires monthly amounts. Review its conversion instructions before entering an amount that is paid on another schedule. Gather the records you plan to use for the displayed income and deduction lines.

  • Wages and similar payments, business income, benefits, retirement payments, interest, rental income, and other recurring income
  • The source description for any income entered as “other”
  • Income-tax deductions, filing status, and dependents claimed
  • FICA or self-employment taxes, Medicare, mandatory dues or retirement payments, and health insurance
  • Court-ordered child support or alimony amounts actually paid, where a listed line applies

This guide does not decide whether a payment belongs in a category or calculate a monthly figure for you.

3. Average monthly expenses

The long form separates monthly expenses into detailed groups. Bills, statements, or another record you already use may help you review the categories without repeatedly stopping:

  • Primary household expenses and any additional household you intend to identify
  • Automobile and alternative-transportation expenses
  • Expenses for children common to both parties
  • Expenses for children from another relationship
  • Insurance and other personal monthly expenses
  • Current monthly creditor payments not listed elsewhere on the affidavit
Estimate reminder: the official form instructs a person to write “estimate” next to an expense amount that does not reflect what the person actually pays currently. Review the PDF for the complete instruction.

4. Assets, debts, and net worth

The asset and liability tables contain separate description, ownership, requested-award or responsibility, marital-value, and nonmarital-value fields. The instructions differ depending on the type of family-law matter. Read those instructions before entering anything in these columns.

  • Financial accounts, investments, digital wallets, virtual currency, real estate, business interests, vehicles, retirement plans, and personal property
  • Mortgages, credit cards, student loans, medical liabilities, vehicle loans, taxes, judgments, and other debts
  • The current values or amounts owed that you independently choose to enter
  • Any separate schedule or balance sheet you intend to use after reviewing the official instructions
Privacy reminder from the official form: list only the last four digits of account numbers. The form also requests an origination date for each student loan listed in the monthly-creditor section.

Sun State cannot classify an asset or debt, decide which column applies, or recommend an award or responsibility selection.

5. Contingent items and child-support worksheet selection

The final financial sections display possible-asset and possible-liability tables and a child-support worksheet selection. Gather any information you plan to consult, but make each description, value, ownership entry, and selection yourself. This checklist cannot determine whether an item is contingent or whether the worksheet selection applies to your case.

6. Contact, review, and signing items

The final pages include service information, certification, date, signature, party contact information, and a nonlawyer-assistance section. Use the official instructions for any signing, service, filing, or e-service step. Sun State leaves execution and nonlawyer-assistance fields blank.

Address safety: the official instructions discuss protected addresses and refer readers to Florida Supreme Court Approved Family Law Form 12.980(h). If address confidentiality may affect you, review those instructions and seek appropriate help before entering an address.
