What the official instructions say this form is for
The instructions describe Form 12.915 as the document used to tell the clerk and the other party about a current mailing address, email address, or a change to either. They state that a new form must be completed, filed, and served when those addresses change.
They also discuss the primary email designation required for an unrepresented party unless excused under the cited court rule. Sun State cannot determine whether an exception applies, whether this form is required at a particular time, or whether another privacy or safety procedure should be used.

1. Match the existing case caption
Organize the judicial circuit, county, case number, division, petitioner, and respondent exactly as they appear in the existing family-law case.
Sun State cannot identify the correct court or case, determine how parties should be labeled, or resolve differences between the filer's information and the clerk's records.
2. Review the mailing-address disclosure
The form asks for the filer's current street address or post-office box, apartment or unit, city, state, ZIP code, telephone number, and fax number. The final page repeats address and contact fields below the signature.
3. Organize the service email addresses
The form provides one primary email line and two optional secondary email lines for serving and receiving documents. The official instructions discuss email service under Florida Rule of General Practice and Judicial Administration 2.516 and say self-represented litigants must serve documents by email unless excused by the clerk.
The instructions also state that when a filer designates email and the other party is represented by an attorney or has also designated email addresses for service, email will be the exclusive means of service. Sun State cannot determine whether that condition or an email-service exception applies, whether an address is appropriate to designate, whether service is effective in a particular situation, or how to comply with the rule's formatting requirements.
4. Complete the certificate only after actual service
The certificate provides selections for email, mail, fax, and hand delivery. It asks for the actual service date and the other party's or attorney's name, address, city/state/ZIP, telephone, fax, and email addresses.
Sun State leaves the certificate blank because it cannot know how, when, or on whom service actually occurred. It does not select a method, identify recipients, calculate deadlines, or determine whether service satisfies current rules.
5. Review the certification and signature label
The form says the filer has read every statement, that each statement is true and correct, and that the statements are made under penalty of perjury. The August 2023 official form labels its signature line “Signature of Petitioner.”
Sun State's configured filler does not add a signature and supports the petitioner label printed on the official form. It cannot determine whether a respondent should use this form or another procedure. Confirm that question with the clerk, court-connected self-help staff, or a licensed Florida attorney before proceeding.
Filing, service, and future updates remain separate steps
The instructions say to file the original with the clerk of the circuit court in the county where the case is filed, keep a copy, and serve a copy on every other party. They state that a new form must be filed and served if a mailing or email address changes.
Completing a PDF does not file it, serve it, confirm receipt, or update the clerk's records. Sun State cannot determine filing procedure, service sufficiency, local requirements, or when another designation is necessary.
