What gets filed, and what gets given to the other party?
That is a summary of the June 2025 source, not a filing decision for any case. Court orders, agreements, exceptions, current rules, and local procedure can affect what a person must do. Do not upload bank statements, tax returns, account numbers, or other sensitive records merely because they appear on the certificate.
What information appears on Form 12.932?
| Official area | Information displayed |
|---|---|
| Case caption | Judicial circuit, county, case number, division, petitioner, and respondent |
| Certifying party | The full legal name of the person making the certificate |
| Section 1 | Service date and listed materials for temporary financial relief only |
| Section 2 | Service date and listed materials for initial, supplemental, and permanent financial relief |
| Certificate of service | Delivery method, service date, and the other party or attorney's contact information |

1. Copy the existing case caption carefully
The first page of the certificate asks for the judicial circuit, county, case number, division, petitioner, and respondent. It then asks for the full legal name of the person certifying compliance.
This guide does not choose a court, determine jurisdiction or venue, assign party roles, or decide whether a name should match another filing.
2. Keep temporary-relief materials separate from the longer list
Section 1 is labeled “For Temporary Financial Relief, Only.” It asks for a service date and displays three principal categories: a short- or long-form financial affidavit; tax returns, an IRS transcript, or listed tax forms in the circumstances printed on the form; and pay stubs or other evidence of earned income for the stated six-month period.
The official instructions describe who serves temporary-relief materials and when. This guide does not determine whether a hearing is for temporary financial relief, identify which items apply, calculate a period, or state that service is timely or complete.
3. Review every Section 2 category against the official form
Section 2 is labeled for initial, supplemental, and permanent financial relief. Each lettered line includes a checkbox and, where applicable, space to identify the materials produced. The categories can be organized as follows:
| Form lines | Printed subject matter | Printed lookback or detail |
|---|---|---|
| a–d | Financial affidavit, tax materials, earned-income evidence, and other income | The form supplies alternatives and six-month or three-year periods for particular lines |
| e–f | Loan and financial applications; deeds, notes, and leases | The form uses 24-month and three-year references for specified records |
| g–j | Bank, brokerage, retirement-plan, and virtual-currency records | The form requests statements and related records for the periods printed on each line |
| k–m | Life insurance, health and dental insurance, and business or trust tax returns | The form lists policy materials, cards, and three tax years for the specified category |
| n–r | Debt and lease records; agreements and parentage records; modification agreements; property-character claims; support orders | Some lines apply only to a stated type of proceeding or claim |
This table is an orientation aid, not a substitute for the form's exact language. Sun State does not decide whether a category applies, whether an account or document falls within a category, whether a record is missing, or whether an item should be produced.
4. State produced documents with specificity
Both sections instruct the signer to check all that apply and state with specificity the documents being produced. They allow additional papers when the printed space is insufficient and say to refer to those papers in the space provided.
This guide does not draft a document description, supply an answer, resolve an objection, or decide whether an attachment is appropriate. Describe only facts the certifying person has independently verified and review the privacy warning before including identifiers.
5. Use the short- or long-form reference only as the court prints it
The instructions refer to Form 12.902(b) for individual gross annual income under $50,000 and Form 12.902(c) for $50,000 or more. They also note that either party may request the long form by serving the appropriate interrogatory form.
Sun State does not calculate gross income, choose a financial affidavit, decide whether a request is valid, or determine whether another form is required. Use the official form and current rules, or seek case-specific help.
6. Protect account numbers and personal information
The Form 12.932 instructions specifically direct the reader to Florida Rule of General Practice and Judicial Administration 2.425 before completing references to account numbers and personal identifying information. The certificate can concern tax, bank, investment, insurance, debt, and family information.
7. Do not certify service before it happens
The certificate-of-service sentence offers email, mail, and hand-delivery checkboxes, followed by a service date. The next page asks for the other party or attorney's name, address, city/state/ZIP, telephone number, and email address.
The packet refers readers to Rule 2.516 and discusses electronic service, Form 12.914, and Form 12.915. This guide does not choose a recipient or delivery method, calculate a service date, determine whether email service was elected, or certify that service occurred.
8. Read the perjury declaration before signing
The last page states under penalties of perjury that the disclosure and statements are complete, accurate, and compliant with Rule 12.285. It asks for the date, party signature, printed name, mailing address, telephone number, and email addresses. The current form does not display a notary block.
Sun State does not sign for a party, verify identity, test the truth or completeness of a certification, or advise anyone to sign. A person should not sign a declaration they do not understand or cannot independently confirm.
9. Note the continuing-duty and nonlawyer instructions
The instructions say that after the listed information and documents have been provided and the certificate filed, the person remains under a continuing duty to promptly give the other party information or documents that change financial status or make earlier information inaccurate. The last page also includes a required information block if a nonlawyer helped complete the form.
This guide does not monitor changes, decide whether information is material or inaccurate, establish when an update is due, or determine what a nonlawyer may lawfully do.
What the official instructions say about timing, exceptions, and agreements
The June 2025 instructions describe a 45-day period following service of an initial petition or supplemental modification petition. They also describe exceptions for simplified dissolution and for the constructive-service circumstance printed in the packet, and say portions of mandatory disclosure may be modified by court order or party agreement while the financial affidavit cannot be waived.
Those statements are source summaries, not a conclusion about any person's deadline or obligations. This guide does not calculate time, determine whether an exception applies, interpret an agreement or order, reconcile the instructions with Form 12.902(k), or advise whether disclosure may be limited.
