When do the official instructions say this affidavit is used?

The October 2023 instructions describe three connected circumstances: a party is requesting establishment or modification of child support; both parents agreed to keep their financial affidavits confidential and completed Form 12.902(k); and each party has already served their financial affidavit on the other party.

Those statements describe the official packet; they are not a Sun State form-selection decision. This guide cannot determine whether the conditions are satisfied, whether a waiver is available, whether an affidavit was complete or properly served, or what a court requires in a particular case.

1. Match the case caption and identify the person signing

The affidavit begins with the judicial circuit, county, case number, division, petitioner, and respondent. The person making the affidavit enters a full legal name and checks petitioner or respondent.

A separate checkbox is provided for unemployment. The printed form directs a person who checks it to explain efforts to find employment on a separate sheet. This guide does not draft that explanation, decide what facts belong in it, or evaluate employment circumstances.

2. Convert nonmonthly amounts before using the numbered lines

The form says all amounts must be monthly. Its instructions provide the following arithmetic for amounts paid on other schedules:

Schedule shownOfficial conversion sequence
HourlyHourly amount × hours worked per week; then × 52; then ÷ 12
DailyDaily amount × days worked per week; then × 52; then ÷ 12
WeeklyWeekly amount × 52; then ÷ 12
Every two weeksBiweekly amount × 26; then ÷ 12
Twice per monthSemimonthly amount × 2

The instructions say expenses may be converted in the same manner. These are the packet's formulas, not a determination of which underlying number, work schedule, time period, or rounding method is correct for any person.

3. Review all sixteen gross-income lines before totaling line 17

The gross-income section displays salary or wages; similar payments such as bonuses, commissions, overtime, and tips; business income; disability benefits or SSI; workers' compensation; unemployment compensation; pension, retirement, or annuity payments; Social Security benefits; two spousal-support lines; interest and dividends; rental income; royalties, trusts, or estates; certain reimbursed expenses and in-kind payments; certain gains from dealing in property; and other recurring income.

The form defines business and rental income in terms of gross receipts minus ordinary and necessary expenses required to produce income. It tells the filer to identify the source of “other” recurring income and to list items included under “other” separately with separate dollar amounts. Line 17 directs the filer to add lines 1 through 16.

This is an index of the printed labels, not financial classification advice. Sun State does not decide whether money belongs on a particular line, whether an expense is ordinary or necessary, whether a gain is nonrecurring, or whether a benefit or payment counts as income.

4. Keep line 18 and its tax detail together

The deductions section starts with federal, state, and local income tax adjusted for the actual filing status, allowable dependents, and tax liabilities. The next displayed fields ask for filing status, number of dependents claimed, federal income tax, and any state and local income tax with the state or locality identified.

This guide does not provide tax advice, choose a filing status, determine allowable dependents, estimate tax liability, or tell a filer which tax figure to use. A qualified tax professional can address tax questions.

5. Read each remaining deduction label narrowly

The remaining printed lines cover FICA or self-employment taxes, Medicare payments, mandatory union dues, mandatory retirement payments, the completing parent's health-insurance premium excluding coverage for the children in the case, the premium that parent pays for those children, court-ordered support for other children that is actually paid, spousal support paid in this marriage, and court-ordered alimony for a prior marriage that is actually paid.

The two court-ordered-payment lines include a place to identify the issuing court. Line 29 directs the filer to add lines 18 through 28. This guide does not determine whether a payment is mandatory, actually paid, court ordered, attributable to a particular person, or otherwise eligible to appear as a deduction.

6. Follow the printed arithmetic without treating it as a support award

Line 30 is labeled present monthly net income and directs the filer to subtract total deductions on line 29 from total gross income on line 17.

That result is one figure on this affidavit. Form 12.902(l) does not itself display the child-support guidelines calculation or issue a child-support order. Sun State does not calculate a guideline amount, recommend support, or predict a court's decision.

7. Review the perjury declaration and contact fields before signing

The last page contains a declaration under penalties of perjury that the signer has read the document and that its facts are true. The signature block asks for the party's signature, printed name, email, address, city/state/ZIP, and telephone number.

Unlike several other family-law affidavits, the reviewed Form 12.902(l) does not display a notary or deputy-clerk block. Completing fields does not sign the declaration, verify the information, file the document, serve it, or create a child-support order.

8. Complete the certificate of service only from what occurred

The certificate asks which listed service methods were used—email, mail, fax, or hand delivery—the date, and the other party or attorney's name and contact information. A separate signature block is provided for the party or attorney, with printed name, emails, address, city/state/ZIP, telephone, fax, and Florida Bar number.

The instructions refer to Rule 2.516, Forms 12.914 and 12.915, and electronic-service requirements. This guide does not select recipients or methods, calculate a service date, determine whether an exception applies, or certify that service occurred.

9. Protect address information when the packet's warning may apply

The special notes warn people in the listed victim-safety circumstances not to enter address, telephone, and fax information at the bottom of the form. The packet instead points to Form 12.980(h), Request for Confidential Filing of Address.

This guide does not assess whether the warning applies or advise how to protect information in a particular case. Use the official court resources or qualified legal help before placing sensitive contact information in a filing.

10. Account for any nonlawyer assistance

The instructions say a nonlawyer who helps complete the form must provide Form 12.900(a), Disclosure from Nonlawyer, before helping and must place identifying information at the bottom of each form assisted with. The last page includes fields for the party choice, individual, business, address, city, state, and telephone number.

Sun State does not tell a customer what legal rights, remedies, defenses, claims, or strategies to pursue. It does not select a form or decide how law applies to a customer's facts.
