Start with the free official packet

The Bay County Clerk currently links a 15-page Small Claims Packet. Form A, the two-page Statement of Claim, appears on packet pages 5 and 6. The remaining pages contain instructions and other official forms. Read the complete packet and check the clerk’s Civil Forms page for an updated copy before preparing Form A.

1. Gather the party and contact information

Form A provides lines for up to two plaintiffs and two defendants, addresses for each side, telephone numbers, and the plaintiff’s email address.

Sun State does not identify a proper plaintiff or defendant, verify a legal name or service address, or decide venue.

2. Separate the five amount fields

The form states that the civil action does not exceed $8,000 and provides separate blanks for the claim amount, interest, attorney’s fees, court cost, and total. Organize the figures you independently intend to enter and confirm what may lawfully be requested before using them.

Arithmetic only: the optional filler can total customer-entered numbers, and the customer can edit that result. It does not calculate damages, interest, fees, recoverable costs, or legal entitlement.

3. Read every printed basis before marking anything

Form A presents ten printed bases and directs the filer to check or circle an appropriate statement. The choices concern an account, goods sold, money loaned, rent due, defective goods or services, labor and materials, a worthless check, an auto collision, breach of contract or warranty, and Other.

Several statements refer to information or documents to list or attach, such as accounts, a promissory note, lease information, a check, time and material charges, or collision facts and damages. Read the exact packet language before making any selection.

Sun State does not classify a dispute, choose a basis, determine whether multiple choices are permitted, draft allegations, or decide which documents should be attached.

4. Prepare your own additional information

The second page of Form A continues the printed basis list and provides an Additional Information area. Gather the dates, transactions, communications, locations, amounts, and documents you independently decide are relevant before writing anything.

Sun State transfers customer-supplied text into the configured form. It does not create, revise, summarize, or assess claim facts or evidence.

5. Leave signatures for manual completion

Form A displays separate date and signature lines for two plaintiffs. The optional filler may place customer-supplied dates in the date blanks but leaves the plaintiff signature lines blank. Clerk, judge, notary, process-server, and agency-only areas elsewhere in the packet also remain untouched.

Sun State does not decide who must sign, provide a signature, notarize, file, or complete any official-use portion of the packet.

6. Confirm current local procedures and fees

The clerk’s Civil Court page identifies small claims as matters up to and including $8,000 in damages, excluding interest, court costs, and attorney fees. The separate Civil Fee Schedule lists filing and issuance charges. Because procedures and fees can change, confirm the current packet and clerk pages before filing.

Optional filler for Form A inside the packet

The clerk’s complete blank packet is free. If you have independently selected Bay County Form A, Sun State’s optional $29 self-service tool transfers your entries into the configured Form A while preserving all official packet pages. It does not choose parties, amounts, claim bases, wording, attachments, filing or service methods, or signing steps.
